Hello and thank you for your participation in this study. The purpose of this training is to familiarize you with the use of a tool for measuring teamwork in the operating room.

We are specifically interested in five elements of teamwork: clinical leadership, team communication, assertiveness, coordination, and respect. You will be trained to rate each element separately and to provide a rating of teamwork overall.
For each teamwork element, we will give a definition and show you the specific behaviors that you will be asked to rate. Please rate each item on a scale of 1 to 5. Unless otherwise noted, one indicates that a behavior "NEVER" occurred during the observation. Three indicates that a behavior occurred "ABOUT HALF" of the time. Five indicates that a behavior "ALWAYS" occurred.
Finally, you will see two brief videos for each teamwork element and have an opportunity to rate them. Your ratings will be recorded for use in our study.
